Who May Need Anti-VEGF Treatment
Certain health conditions and lifestyle factors raise the risk of developing a retinal disease that requires anti-VEGF therapy. Understanding these risk factors can help you and your Retina Specialist stay ahead of vision-threatening changes.
Wet AMD primarily affects people over age 50 and is responsible for the majority of AMD-related vision loss that leads to legal blindness. Risk factors include smoking, a family history of AMD, cardiovascular disease, and high blood pressure. Wet AMD develops when abnormal blood vessels grow beneath the macula, the central portion of the retina responsible for sharp, detailed vision.
Approximately one-third of all people with diabetes have some form of diabetic retinopathy, and among those patients, a significant portion develop vision-threatening disease. Uncontrolled blood sugar, high blood pressure, and a longer duration of diabetes all increase the likelihood of needing anti-VEGF treatment. Diabetic macular edema, the most common cause of vision loss in working-age adults with diabetes, responds well to this therapy when diagnosed and treated early.
Retinal vein occlusion, a blockage of the small veins draining the retina, is more common in people with high blood pressure, diabetes, and glaucoma. Myopic choroidal neovascularization can occur in younger adults with severe nearsightedness. In general, conditions that put stress on blood vessels throughout the body, including smoking, obesity, and cardiovascular disease, also raise the risk of retinal conditions that may call for anti-VEGF therapy.
Anti-VEGF Medications We Use
Several anti-VEGF medications are available, and our Retina Specialists select the option best suited to each patient's diagnosis, treatment history, and lifestyle. All of these agents work by blocking VEGF, but they differ in molecular design, duration of effect, and approved uses.
Multiple anti-VEGF drugs are currently approved for retinal use in the United States. Our team will discuss which option makes the most sense for your specific condition.
- Lucentis (ranibizumab): The first anti-VEGF medication designed specifically for use in the eye. It is approved for wet AMD, diabetic retinopathy, diabetic macular edema, retinal vein occlusion, and myopic choroidal neovascularization, typically given every four weeks.
- Eylea (aflibercept): One of the most widely used anti-VEGF agents, approved for wet AMD, diabetic macular edema, diabetic retinopathy, and retinal vein occlusion. After an initial series of monthly injections, dosing may extend to every four to eight weeks.
- Eylea HD (high-dose aflibercept): A higher-concentration formulation of aflibercept designed to allow longer intervals between injections. After initial monthly doses, some patients may be able to go eight to sixteen weeks between treatments.
- Vabysmo (faricimab): A bispecific antibody that targets two proteins, VEGF-A and angiopoietin-2, which both contribute to abnormal vessel growth and leakage. It is approved for wet AMD and diabetic macular edema, and many patients in clinical trials were able to extend their dosing interval to four months.
Avastin (bevacizumab) was originally developed and approved as a cancer treatment. It is not FDA-approved for use in the eye, but it is widely used off-label by Retina Specialists around the world to treat wet AMD, diabetic macular edema, and retinal vein occlusion. Large clinical trials have shown that Avastin produces visual outcomes comparable to other approved agents for wet AMD. Your Retina Specialist will discuss whether Avastin is an appropriate option for your situation.
Biosimilars are medications that are highly similar to an already-approved biologic drug. Two biosimilars of Lucentis have been approved in the United States, Byooviz and Cimerli, both carrying the same approved indications as the original medication. These options expand the choices available to patients and their care team when selecting a treatment plan.
What to Expect During Treatment
Many patients feel nervous before their first injection, but most find that the experience is much more comfortable than they expected. Understanding each step of the process can help ease any anxiety before your appointment.
Before your first injection, your Retina Specialist will perform a thorough examination that typically includes optical coherence tomography, or OCT, a scan that produces detailed cross-sectional images of your retina. Fluorescein angiography, a test using dye and a specialized camera to photograph blood flow in the retina, may also be performed. These imaging tools confirm the diagnosis and guide every treatment decision we make.
On the day of the injection, your eye will be numbed using anesthetic drops or a small injection of numbing medicine. The surface of the eye is then carefully cleaned with an antiseptic solution to minimize the risk of infection.
The actual injection takes only a few seconds. A very fine needle delivers a small amount of medication through the white part of the eye and into the vitreous cavity. Most patients feel a sense of pressure rather than sharp pain, and you typically will not see the needle. Mild redness, a gritty feeling, or minor irritation in the treated eye is common for a day or two after the injection. Some patients notice small floaters or a temporary bubble in their vision, which usually resolves within a few days. Anti-VEGF treatment typically begins with a loading phase of monthly injections for the first two to three months. After that, the schedule is guided by how your eye responds. Many Retina Specialists use a treat-and-extend approach, gradually increasing the time between injections as long as the retina remains stable. Some patients are eventually able to go several months between treatments, while others need more frequent injections to maintain control of their condition.
Anti-VEGF therapy is most often an ongoing commitment rather than a short course of treatment. Sticking with your recommended schedule is one of the most important things you can do to protect your long-term vision.

Anti-VEGF therapy has transformed the outlook for patients with serious retinal disease. Understanding what the treatment can realistically achieve and why consistency matters will help you get the most benefit from your care.
Anti-VEGF treatment improves vision in roughly one out of three patients who receive it. For the large majority, approximately nine out of ten, it stabilizes vision and prevents further decline. Before anti-VEGF therapy became available, most people with wet AMD experienced progressive and irreversible central vision loss. Today, patients have a much better chance of preserving the central vision they need to read, drive, and recognize faces.
The benefits of anti-VEGF therapy depend heavily on receiving injections on schedule. Missing appointments or stopping treatment early can allow the disease to become active again. When that happens, fluid and abnormal vessels may return, and vision gains can be partially or fully lost. Living Well With Ongoing Treatment
Managing a chronic retinal condition involves more than office visits and injections. There are steps you can take between appointments to monitor your vision, support your eye health, and make the treatment routine more manageable.
Between appointments, we encourage patients to check their vision using an Amsler grid. This is a simple chart of straight horizontal and vertical lines with a dot at the center. Looking at the grid one eye at a time can reveal changes in central vision such as wavy lines, missing areas, or new dark spots. Your Retina Specialist can provide an Amsler grid and show you how to use it correctly.
Receiving regular eye injections can feel overwhelming, especially at first. It is completely normal to feel anxious before early appointments. Most patients find that once they have been through the procedure, the anticipation is harder than the injection itself. If anxiety is a concern for you, let your Retina Specialist know.
