CRVO most commonly affects people aged 50 and older. The vast majority of cases occur in patients over the age of 55, and the risk increases meaningfully with each decade of life. CRVO affects roughly 0.8 out of every 1,000 people in the developed world, making it one of the more common retinal vascular conditions we treat.
High blood pressure is the most frequently associated health condition in CRVO patients, present in nearly three-quarters of those over the age of 50 who develop the condition. Elevated blood pressure damages the walls of blood vessels throughout the body, including those in the retina, and significantly increases the risk of a venous blockage.
Diabetes also raises CRVO risk. High blood sugar levels cause ongoing damage to blood vessel walls over time, making them more vulnerable to blockage and leakage. Keeping both blood pressure and blood sugar well controlled is one of the most important things a patient can do to reduce their overall risk.
Several additional conditions and lifestyle factors can increase the likelihood of developing CRVO. These include:
- Glaucoma, a condition involving elevated pressure inside the eye
- Atherosclerosis, or hardening and narrowing of the arteries
- Blood clotting disorders that cause blood to clot more easily than normal
- High cholesterol
- Smoking
- Use of oral contraceptives, which is the most common underlying association with CRVO in younger women

Recognizing the Symptoms
CRVO can develop suddenly or worsen gradually over hours or days. Knowing what to watch for helps ensure you get care as quickly as possible, which matters greatly for the outcome.

Diagnosis and Testing

Your Retina Specialist will begin with a comprehensive eye examination. This includes measuring your visual acuity to assess how clearly you can see, checking the pressure inside your eye, and dilating your pupil with special eye drops so the retina can be examined in detail. During dilation, the specialist looks for characteristic signs of CRVO such as swollen retinal veins, widespread bleeding throughout the retina, and fluid buildup in the macula.
Optical coherence tomography, known as OCT, is a non-invasive imaging test that produces detailed cross-sectional pictures of the retinal layers. OCT allows us to measure the exact amount of fluid and swelling in the macula and to track how the retina responds to treatment over time. OCT angiography can also show blood flow patterns in the retinal vessels without the need for an injected dye.
Fluorescein angiography is another key diagnostic tool. A special dye is injected into a vein in your arm, and a camera captures rapid images as the dye moves through the retinal blood vessels. This test reveals areas of blocked circulation and vessel leakage, helping us determine whether your CRVO is ischemic or non-ischemic. We also use wide-field imaging and fundus photography to document the full extent of the condition and track changes at every visit.
Because CRVO is closely linked to systemic conditions, your Retina Specialist may recommend blood work and a cardiovascular evaluation in coordination with your primary care provider. Treatment Options for CRVO
Treatment focuses on reducing swelling in the macula, preserving vision, and preventing serious complications. The right approach depends on the type and severity of your CRVO, your overall health, and how your eye responds to initial therapy.
Anti-VEGF injections are the primary treatment for macular edema caused by CRVO. VEGF, or vascular endothelial growth factor, is a protein that drives abnormal blood vessel growth and fluid leakage in the retina. Anti-VEGF medications block this protein to reduce swelling and help stabilize or improve vision.
Several anti-VEGF medications are used to treat CRVO-related macular edema, including Lucentis (ranibizumab), Eylea (aflibercept), and Avastin (bevacizumab). Treatment typically begins with a series of monthly injections, and the frequency is adjusted based on how your eye responds. Many patients require ongoing injections to maintain the benefits of treatment.
Vabysmo (faricimab) is a newer bispecific antibody that targets both VEGF and a second protein called angiopoietin-2, which also plays a role in blood vessel stability and leakage. Clinical studies have shown that Vabysmo performs comparably to established anti-VEGF therapies for CRVO and may allow some patients to extend the time between injections after an initial treatment phase.

For patients who do not respond adequately to anti-VEGF injections, or who have specific medical reasons that limit their use, intravitreal steroid therapy may be recommended. A dexamethasone implant injected into the eye slowly releases medication over several months, reducing inflammation and swelling in the retina. This approach was among the first intravitreal treatments specifically approved for macular edema from retinal vein occlusion.
Steroid therapy does carry some risks that require monitoring. These include a rise in eye pressure and, in some patients, acceleration of cataract formation. Your Retina Specialist will weigh these considerations carefully when determining whether steroid therapy is appropriate for you.
Panretinal photocoagulation, or PRP, is a laser treatment used in severe cases of ischemic CRVO. When large areas of the retina lose their blood supply, the eye may respond by producing abnormal new blood vessels, a process called neovascularization. These fragile vessels can bleed and lead to serious complications including neovascular glaucoma.
PRP applies a thermal laser to areas of the retina that have lost circulation, reducing the signals that trigger abnormal vessel growth. PRP does not restore lost central vision, but it plays an important role in preventing the serious complications that can result from ischemic CRVO.
What to Expect During and After Treatment

Most patients with CRVO receive intravitreal injections, meaning medication is injected directly into the eye. Before each injection, the eye is numbed with anesthetic drops and cleaned with an antiseptic solution. The injection takes only a few seconds. Living With CRVO

Because CRVO is so strongly linked to cardiovascular risk factors, working with your primary care doctor to control blood pressure, blood sugar, and cholesterol is a core part of your ongoing care. Smoking significantly raises vascular risk, and stopping is one of the most impactful changes a patient can make.
Younger women who develop CRVO while using oral contraceptives should discuss this with both their Retina Specialist and their prescribing physician. Oral contraceptive use is the most common underlying association with CRVO in this age group, and continuing them after a venous occlusion carries additional risk.
