Diabetic Retinopathy: Protecting Your Vision

Who Is at Risk

Anyone living with diabetes can develop diabetic retinopathy, regardless of how well they manage the condition. Certain factors increase the likelihood of developing it or cause it to progress more quickly. Knowing your personal risk helps you and your care team stay ahead of the disease.

Diabetic retinopathy can affect people with type 1, type 2, or gestational diabetes (a form of diabetes that can develop during pregnancy). Risk rises with the length of time a person has had diabetes, and over time more than half of people with diabetes develop some degree of retinal damage.

Several factors increase the chance that diabetic retinopathy will develop or worsen. Poor blood sugar control and a longer duration of diabetes carry the strongest association, but other systemic and lifestyle factors also play a meaningful role.

  • Long duration of diabetes
  • Poorly controlled blood sugar levels
  • High blood pressure
  • High cholesterol
  • Tobacco use
  • Pregnancy in women with pre-existing diabetes

Addressing as many of these factors as possible, in partnership with your primary care provider and retina specialist, can meaningfully slow the progression of retinal disease.

Women with diabetes who become pregnant face a higher risk for diabetic retinopathy. Hormonal changes and blood sugar fluctuations during pregnancy can accelerate retinal damage even if the condition was previously mild or stable. A dilated eye exam should be scheduled as early as possible during pregnancy, and monitoring should continue throughout. Your retina specialist will recommend an appropriate follow-up schedule based on your current level of retinal involvement.

Recognizing the Symptoms

One of the most dangerous aspects of diabetic retinopathy is that it can cause significant retinal damage before you notice any change in your vision. Understanding which symptoms to watch for, and which require immediate attention, can make the difference between preserving and losing your sight.

In its earliest stages, diabetic retinopathy typically causes no pain and no visible changes to your vision. Some people may notice mild difficulty reading or occasional blurriness that comes and goes, but these changes are easy to dismiss. This is why routine eye exams are essential even when your eyes feel completely normal.

As diabetic retinopathy advances, symptoms become more noticeable. Bleeding into the vitreous can cause dark, floating spots or thread-like streaks drifting across your field of vision. Vision may become blurry or distorted, particularly if the macula is involved. Colors may appear washed out or dull, and reading or fine detail work may become increasingly difficult.

Certain changes in vision indicate a possible emergency and require same-day evaluation. Do not wait for a routine appointment if you notice any of the following symptoms.

  • A sudden increase in floaters or dark spots in your vision
  • Flashes of light in one or both eyes
  • A dark curtain or shadow moving across any part of your visual field
  • Sudden, severe vision loss in one eye

These symptoms may indicate a vitreous hemorrhage (bleeding inside the eye) or a retinal detachment, both of which require prompt treatment to prevent permanent vision loss.

Diagnosis and Imaging

Detecting diabetic retinopathy early depends on thorough, specialized testing. Our team uses a combination of clinical examination and advanced imaging to assess the health of your retina, track changes over time, and guide treatment decisions. We perform serial retinal photo comparisons at every visit so that even subtle changes are documented and caught early.

A comprehensive dilated eye exam is the foundation of diabetic retinopathy diagnosis. Special eye drops widen your pupils so your retina specialist can see the retina clearly. During the exam, we look for leaking blood vessels, retinal swelling, fatty deposits, abnormal new vessel growth, and scar tissue. Because the disease can be present with no symptoms at all, a dilated exam is the only reliable way to detect it in its earliest and most treatable stages.

Optical coherence tomography, or OCT, is a non-invasive imaging test that produces detailed cross-sectional images of the retina. It measures retinal thickness and identifies fluid buildup within the macula, making it especially valuable for diagnosing and monitoring diabetic macular edema. OCT angiography can also map blood flow in the retina without the need for dye. The test is painless and takes only a few minutes.

Fluorescein angiography maps blood flow through the retinal vessels using a special yellow dye injected into a vein in your arm. As the dye travels through your eye's blood vessels, a camera captures rapid sequential photographs. This test reveals areas of leakage, blockage, or abnormal vessel growth that may not be visible through other methods. It helps us precisely plan laser treatment and assess how blood vessels are functioning throughout the retina.

Wide-field fundus imaging captures a broader view of the peripheral retina than standard photography, which is especially useful when abnormal vessels or retinal damage extend beyond the central area. B-scan ultrasound can be used when bleeding or other media opacity blocks the view of the retina, helping us assess retinal structure even when the inside of the eye cannot be seen directly.

Treatment Options

Treatment for diabetic retinopathy has advanced considerably, and several effective options are available depending on the stage and severity of your condition. Our retina specialists tailor every treatment plan to your specific situation, combining approaches when needed to achieve the best possible outcome for your vision.

Anti-VEGF injections are the most widely used treatment for both diabetic retinopathy and diabetic macular edema. These medications are delivered directly into the vitreous cavity of the eye and work by blocking the VEGF chemical signal that drives abnormal vessel growth and fluid leakage. Research consistently shows that anti-VEGF therapy leads to better vision outcomes than laser treatment alone for many patients with DME.

Several anti-VEGF medications are used in our practice, including Avastin (bevacizumab), Lucentis (ranibizumab), Eylea (aflibercept), and Vabysmo (faricimab). Each has a distinct dosing schedule and mechanism, and your retina specialist will recommend the most appropriate option based on your condition and treatment history.

Retinal laser photocoagulation uses a focused thermal laser to treat areas of the retina affected by diabetic disease. Panretinal photocoagulation, or PRP, applies a series of small laser burns to the peripheral retina to reduce oxygen demand and suppress abnormal vessel growth in proliferative disease. Focal laser treatment can be applied to specific leaking vessels near the macula. While anti-VEGF injections are now the preferred first-line approach for DME, laser therapy remains an important tool, particularly for advanced proliferative disease.

Vitrectomy is a surgical procedure to remove the vitreous gel from inside the eye. It is typically recommended when significant bleeding into the vitreous does not clear on its own, or when scar tissue is pulling on the retina and threatening to cause a detachment. During the procedure, the vitreous is removed and replaced with a saline solution or a gas bubble. Vitrectomy is performed at an outpatient surgical center by our retina specialists and may be combined with laser treatment or membrane removal during the same procedure.

Steroid implants are an option for certain patients with DME, particularly those who have not responded adequately to anti-VEGF treatment. The Iluvien implant delivers a sustained, low dose of corticosteroid to the back of the eye over an extended period. These therapies are selected carefully, as steroids can raise eye pressure and accelerate cataract formation in some patients. Your retina specialist will discuss whether a steroid-based approach is appropriate for your individual situation.

What to Expect During Treatment

Starting treatment for diabetic retinopathy can feel unfamiliar, and many patients have questions about what the process involves. Understanding what happens during an injection, how often visits are needed, and what realistic outcomes look like can help you feel more confident and prepared throughout your care.

Intravitreal injections are performed in our office in a quick, well-established procedure. The eye is numbed with anesthetic drops, and the surface is carefully cleaned with an antiseptic solution before the injection is given. The injection itself takes only a few seconds. Most patients feel a sensation of pressure rather than sharp pain. Mild redness, irritation, or temporary floaters after the injection are common and typically resolve within a few days.

Most anti-VEGF treatment plans begin with a loading phase of monthly injections, followed by a gradual extension of the interval between visits based on how your retina responds. Some newer medications allow treatment intervals of every 8 to 16 weeks once the disease is stabilized. We monitor your progress with regular OCT imaging and dilated exams at each visit. Keeping all scheduled appointments is essential for maintaining the gains made through treatment.

Treatment can stabilize vision and, in many cases, improve it. However, outcomes vary depending on how much damage the retina has already sustained and how quickly treatment was started. Some retinal damage may not be fully reversible. The primary goal of treatment is to prevent further vision loss and preserve as much functional vision as possible, and early intervention consistently leads to better long-term results.

Living With Diabetic Retinopathy

Managing diabetic retinopathy is not only about the treatments you receive in our office. What you do between visits matters just as much. Systemic disease control, regular monitoring, and adjustments to daily habits all play a significant role in protecting your long-term vision.

Keeping blood sugar, blood pressure, and cholesterol within recommended ranges is the most powerful step you can take to slow retinal disease progression. Even modest improvements in blood sugar control have been shown to reduce the rate of progression and lower the risk of vision-threatening complications. Work closely with your primary care provider or endocrinologist to establish and maintain your personal target ranges.

Because diabetic retinopathy can advance without causing noticeable symptoms, regular dilated eye exams are non-negotiable for anyone with diabetes. Most guidelines recommend at least one dilated exam per year, though your retina specialist may recommend more frequent visits depending on the severity of your condition. Consistent follow-up is the most reliable way to catch changes before they threaten your vision.

Eating a balanced diet, exercising regularly, maintaining a healthy weight, and quitting tobacco all contribute to better blood sugar control and reduced vascular risk throughout the body, including in the eyes. If you have experienced some degree of vision loss, low-vision aids such as magnifying lenses, large-print materials, and improved task lighting can help you continue daily activities more comfortably.

A diagnosis of diabetic retinopathy can bring real anxiety about your vision and your future. Those feelings are understandable and valid. Resources from organizations such as the American Diabetes Association and the BrightFocus Foundation offer education, community, and practical guidance. Staying engaged in your care plan and asking questions at every visit helps you remain an active partner in protecting your sight.

Treatment can reduce retinal swelling, stop abnormal vessel growth, and improve vision in many patients, but not all retinal damage can be undone. The extent of recovery depends largely on how early the condition is detected and how consistently it is treated. Patients who begin treatment before significant structural damage has occurred tend to have the best outcomes. This is why annual exams are so important even when your vision feels normal.

It is common for diabetic retinopathy to develop in both eyes, though one eye may be more severely affected than the other. The condition does not always progress at the same rate in each eye. For this reason, both eyes are examined and imaged at every visit. Treatment plans are often tailored separately for each eye based on the degree of disease present, so do not assume that the same approach applies to both sides.

Without treatment, diabetic retinopathy can advance to complications that are far more difficult to manage. Proliferative disease can lead to vitreous hemorrhage (significant bleeding inside the eye), traction retinal detachment (where scar tissue pulls the retina away from the back of the eye), and neovascular glaucoma (dangerously elevated eye pressure caused by abnormal vessel growth on the iris). These conditions often require more complex surgery and carry a higher risk of permanent vision loss than earlier-stage disease. Early intervention is always preferable to managing advanced complications.

There is no guaranteed way to prevent diabetic retinopathy if you have diabetes, but your choices can meaningfully lower your risk. Tight control of blood sugar, blood pressure, and cholesterol reduces the rate of disease onset and progression. Not smoking removes an additional vascular risk factor. Scheduling a dilated eye exam as soon as you are diagnosed with diabetes, rather than waiting for symptoms to appear, gives you and your retina specialist the earliest possible window for intervention.

If your vision has been gradually changing or you are due for a routine follow-up, scheduling a standard appointment is appropriate. However, sudden changes in vision require same-day evaluation. A sudden increase in floaters, new flashes of light, a dark curtain or shadow moving across your vision, or any abrupt loss of vision in one eye should be treated as urgent. Anti-VEGF injections have been used in retinal care for many years and have a well-established safety profile when performed by trained specialists under sterile conditions. Most patients tolerate a long-term injection schedule well, and many experience extended intervals between treatments over time as their condition stabilizes.
