Conditions We Evaluate With Fluorescein Angiography

Our Retina Specialists use this test across a broad range of retinal conditions. The images it produces help us understand the type, location, and severity of disease so that we can make well-informed treatment decisions for each patient.

Age-related macular degeneration (AMD) is one of the most common reasons we order this test. AMD affects the macula, the central part of the retina responsible for sharp, detailed vision. Fluorescein angiography is especially useful in identifying wet AMD, a form of the disease where abnormal blood vessels grow beneath the retina and leak fluid or blood. The images help our team determine the precise location and extent of disease activity.

Diabetic retinopathy is damage to the small blood vessels in the retina caused by diabetes. Fluorescein angiography can reveal areas of leakage, blocked circulation, and new abnormal vessel growth. These findings help our Retina Specialists determine the stage of the condition and whether anti-VEGF injections or laser photocoagulation (a targeted laser treatment) are needed.

A retinal vein occlusion occurs when a vein that carries blood away from the retina becomes blocked. This can cause swelling, bleeding, and significant vision loss. Fluorescein angiography shows exactly where the blockage is and how much of the retina has been affected, which guides our approach to treatment.

Fluorescein angiography is also used to evaluate a number of additional conditions that affect the back of the eye.

  • Macular edema, which is swelling in the central retina that blurs and distorts vision
  • Uveitis, which is inflammation inside the eye
  • Central serous chorioretinopathy, a condition where fluid collects beneath the retina
  • Vascular abnormalities or malformations in the retina
  • Certain retinal tumors or growths

How to Prepare for Your Appointment

A little preparation before your visit helps the test go smoothly and ensures our team has everything they need to keep you safe. Most preparation is straightforward, but a few details are especially important to share ahead of time.

Before the test, let your Retina Specialist know about any allergies you have, particularly if you have ever had a reaction to fluorescein dye. If you are also having indocyanine green angiography (a related imaging test that uses a different dye), tell us if you have an allergy to iodine or shellfish, since that dye contains iodine. Also share any history of pregnancy, heart conditions, or kidney problems, as these may factor into how we plan or interpret the test.

Pregnancy is considered a relative reason to avoid fluorescein angiography. In certain situations where the information is essential, the test may still be performed safely, and your Retina Specialist will discuss the risks and benefits with you directly.

Because your pupils will be dilated during this visit, your vision will be temporarily blurry afterward and your eyes will be sensitive to bright light. Plan to have someone drive you home. Wearing sunglasses for the ride back is helpful. Wear comfortable clothing with sleeves that can be rolled up easily, since the dye is injected into a vein near the inside of your elbow.

What Happens During the Procedure

The procedure takes place in our office and is performed by our trained clinical staff. It is not a surgical procedure, and most patients find it straightforward and well-tolerated. Your Retina Specialist or a certified ophthalmic technician will walk you through each step.

The process follows a consistent sequence from start to finish.

  • Eye drops are placed in your eyes to dilate (widen) your pupils
  • You sit in front of a specialized retinal camera, and baseline photographs are taken before any dye is used
  • A small amount of sodium fluorescein dye is injected into a vein in your arm
  • As the dye circulates to the blood vessels in your eye, the camera takes a rapid series of photographs over several minutes
  • Later images may be captured five to ten minutes after the injection to show how the dye drains from the retinal vessels

The injection feels similar to a routine blood draw. You may notice a brief sting or pressure when the needle enters the vein. Some patients experience a warm sensation as the dye enters the bloodstream. During the photography phase, bright flashes of light from the camera are normal. These flashes may cause your vision to appear temporarily dark or slightly tinted for a few minutes, but this effect fades quickly on its own.

After the Test

Once the procedure is complete, there are a few things to expect as the dye works its way out of your system. Most effects are mild and temporary. Our team will review what to watch for before you leave.

After the test, the dye will temporarily change the color of your skin and urine. Your skin may take on a slightly yellow tone for a few hours, and your urine will likely appear orange or dark yellow for up to 24 hours. These changes are harmless and go away on their own as your kidneys clear the dye from your body. Drinking plenty of water after the test helps speed this process along.

Some patients experience mild nausea during or shortly after the injection. This typically passes within a few minutes. Eating a light meal before your appointment may help reduce this effect.

Serious reactions to fluorescein dye are uncommon, but they can occur. Our clinical staff is trained to recognize and respond to any reaction quickly, and emergency medications and equipment are always available in the office during the procedure. Possible reactions include the following.

  • Hives or itchy skin
  • Swelling at the injection site
  • Vasovagal responses such as fainting or a temporary drop in heart rate
  • Difficulty breathing, which is very rare

If you have had any prior reaction to fluorescein dye, it is essential to share that history with your Retina Specialist before the test is performed.

Understanding Your Results

Your Retina Specialist will review the angiography images and explain what they show. Understanding the difference between normal and abnormal findings can help you follow the conversation and ask better questions at your visit.

In a normal fluorescein angiogram, the dye fills the retinal arteries and veins in a smooth, predictable pattern and then drains away without pooling or leaking. The blood vessels appear sharp and well-defined, with no areas of unexpected brightness or shadow. Normal results suggest that retinal blood flow is healthy and functioning as expected.

Abnormal findings can appear in several ways. Dye leaking from blood vessels may point to inflammation, swelling, or wet AMD. Dark areas where the dye does not reach may suggest blocked circulation, which can occur in diabetic retinopathy or retinal vein occlusion. Bright areas of new dye collection may indicate neovascularization, the growth of abnormal new blood vessels.

Your Retina Specialist will explain what the findings mean for your specific situation. The results often determine whether treatment is needed, which type is most appropriate, and how soon it should begin.

Advances in Retinal Angiography

Retinal imaging technology continues to evolve, giving our team more ways to evaluate your eye health with greater detail and, in some cases, less discomfort. Our practice uses the imaging approach that best fits each patient's clinical needs.

Standard fluorescein angiography focuses on the central retina. Ultra-widefield fluorescein angiography (UWFFA) uses advanced camera technology to photograph a much larger area, including the far outer edges of the retina. This broader view can detect disease in regions that standard imaging may not capture, which has changed how some patients are diagnosed and treated.

In oral fluorescein angiography, the dye is swallowed rather than injected. This approach carries a lower risk of allergic reactions and may be considered for patients who have difficulty with intravenous access or who are at higher risk for injection-related reactions. Mild stomach discomfort is the most commonly reported side effect with this method.

Optical coherence tomography angiography (OCTA) is a newer imaging technology that maps retinal blood flow in three dimensions without any dye injection at all. While OCTA is completely noninvasive, it does not show dye leakage the way traditional fluorescein angiography does. Many Retina Specialists, including our team, use both tests together to get the most complete picture of retinal health.

When Should You See a Retina Specialist?

Knowing when to reach out about your vision can make a meaningful difference in outcomes. Whether you are due for routine monitoring or experiencing something new, our team is here to help you navigate next steps.

If you have a known retinal condition such as AMD, diabetic retinopathy, or retinal vein occlusion, your Retina Specialist may recommend fluorescein angiography at regular intervals. Staying on your recommended schedule helps catch any progression early, when treatment tends to be most effective.

Contact a Retina Specialist promptly if you notice changes in your vision. These include blurry central vision, distortion of straight lines, dark spots in your visual field, or difficulty seeing in low light. These symptoms may indicate that your existing condition is changing or that a new problem has developed.

Some symptoms require immediate attention and should not wait for a scheduled appointment. Seek care right away if you experience a sudden increase in floaters, new flashes of light, a shadow or curtain spreading across your vision, or sudden vision loss in one eye. These may be signs of a retinal tear, retinal detachment, or another sight-threatening emergency.

Yes, fluorescein angiography is considered safe for people with diabetes, and it is one of the most important tools for evaluating diabetic retinopathy. The dye does not interact with insulin or diabetes medications. However, diabetes is sometimes accompanied by kidney disease, which can affect how efficiently the body clears the dye. Your Retina Specialist will factor in your overall health history when recommending the test, and the timing may be adjusted if needed.

OCT (optical coherence tomography) creates detailed cross-sectional images of the retinal layers, showing their thickness and structure. Fluorescein angiography focuses on blood flow, revealing whether vessels are leaking, blocked, or growing abnormally. OCT requires no dye and is completely noninvasive. Because each test captures different information, your Retina Specialist may order both to build a complete picture of your condition.

Frequency depends on your diagnosis and how your condition responds to treatment. Some patients need the test only once to confirm a diagnosis. Others may have it repeated every few months or annually to track changes over time. Your Retina Specialist will set a schedule based on your individual situation, and noninvasive alternatives like OCTA may be used when appropriate to reduce the need for repeated dye injections.

Most patients do not need to fast before fluorescein angiography. Eating a light meal beforehand may actually help reduce the mild nausea that some people experience after the dye injection. Follow any specific instructions your Retina Specialist provides, since recommendations may vary based on your health history or other procedures being done at the same visit.

Always disclose a prior reaction to your Retina Specialist before the test. Depending on how mild or severe your previous reaction was, your care team may recommend pretreatment with antihistamines or corticosteroids to reduce risk. In some cases, an alternative imaging approach such as OCTA may be used instead. Our staff is equipped to manage reactions if they do occur, but sharing your history in advance is the most important step you can take.

Your vision will be temporarily blurry after the test due to pupil dilation, not the dye itself. This effect typically lasts a few hours. The bright camera flashes used during the procedure may also leave your vision appearing temporarily dark or slightly tinted, but this fades within minutes. Driving yourself home is not safe while your pupils are dilated, so please arrange a ride in advance.
