A new blind spot accompanied by flashes of light, a sudden shower of new floaters, or a dark curtain or shadow spreading across your peripheral vision should be evaluated the same day. These combinations suggest a retinal tear or detachment, which is a sight-threatening emergency. Do not wait to see if these symptoms improve on their own.
A new scotoma that appeared suddenly and is growing larger, becoming darker, or is paired with new distortion of straight lines also warrants rapid evaluation. This pattern can reflect wet AMD, active fluid accumulation under the macula, or a vascular event in the retina. A scotoma that has been present for months or years, has been previously examined, and has not changed does not require an emergency visit. Every human eye has a natural blind spot corresponding to the optic disc, and this is entirely normal. A scotoma from a previously treated and healed retinal condition that remains exactly the same at each visit can be followed on a scheduled basis. That said, any change in a known scotoma, including growth, darkening, or new symptoms alongside it, should be reported to your care team without delay.

This allows direct visualization of the fovea for signs of a macular hole, epiretinal membrane, subretinal fluid, or photoreceptor damage. The peripheral retina is also examined for tears, thinning, or other findings that may be contributing to your symptoms.
Optical coherence tomography (OCT) produces detailed cross-sectional images of the retinal layers and is the single most valuable tool for evaluating a persistent scotoma. It can reveal a macular hole as a clear defect through the foveal tissue, subretinal fluid from CSC as a distinct pocket beneath the retina, or the abnormal blood vessel growth of wet AMD. PAMM appears as a bright band within a specific deep retinal layer, while geographic atrophy shows loss of the outer photoreceptor layers. The OCT pattern guides both the diagnosis and the treatment approach.
Visual field testing maps the exact location and extent of the blind spot within your field of vision. Microperimetry is a more specialized version of this test that overlays visual sensitivity measurements directly onto a retinal image, precisely matching the functional scotoma to the underlying structural abnormality. The Amsler grid, a simple pattern of straight lines with a center dot, is a practical tool for detecting and tracking both scotomas and areas of distortion at home and in the office.
When vascular leakage or reduced blood flow is suspected, additional imaging provides important detail. Fluorescein angiography uses a safe dye to reveal leaking blood vessels in wet AMD or CSC and areas of blocked flow in vascular occlusions. OCT angiography provides a non-invasive map of the retinal capillary layers and can detect the deep vessel dropout that characterizes PAMM. Fundus autofluorescence imaging highlights patterns of retinal pigment epithelium damage in AMD and related conditions. Treatment Options for a Persistent Blind Spot
The right treatment depends entirely on the underlying cause of the scotoma. Some conditions call for surgery, others respond to injections or laser therapy, and some require careful monitoring combined with strategies to make the most of remaining vision.
Full-thickness macular holes are treated with a surgical procedure called pars plana vitrectomy. During surgery, the vitreous gel is carefully removed from the eye, and any membrane pulling on the macula is gently peeled away. A small gas bubble is then placed inside the eye to press against the macular surface and support hole closure. Patients maintain a face-down position for a period after surgery to keep the bubble in proper contact with the hole. Success rates for macular hole closure are high, and many patients experience meaningful improvement in their central scotoma, especially when the hole is addressed before it becomes large or longstanding.
Acute CSC is typically managed with observation, since the fluid often resolves on its own within a few months. If corticosteroid medications are contributing to the problem, reducing or stopping them under the guidance of the prescribing physician may help. For chronic or recurring CSC that does not resolve independently, photodynamic therapy using a medication called verteporfin can reduce fluid recurrence. For blind spots caused by conditions that have run their course, such as resolved CSC, healed PAMM, or stable geographic atrophy, the focus shifts to careful monitoring and practical support. Serial OCT and visual field testing track whether the scotoma is changing over time. Low-vision rehabilitation teaches eccentric viewing techniques, which train the eye to use the area of the retina just beside the scotoma for tasks that require sharp central vision. Magnification aids, improved lighting, and electronic assistive devices can all help reduce the daily impact of a fixed blind spot.

Most of the conditions described here affect only one eye, which is why patients sometimes do not notice the scotoma until they cover their good eye.
