When Is Fluctuating Vision an Emergency?
Some patterns of fluctuating or transient vision loss signal a serious underlying problem that cannot wait for a routine appointment. Knowing which symptoms require immediate attention can make a meaningful difference in outcomes.

- Brief, complete loss of vision in one eye, even if vision fully returns
- Sudden blurriness accompanied by new floaters and flashes of light
- A dark curtain or shadow appearing in any part of your visual field

The last combination of symptoms may indicate giant cell arteritis, a vascular condition that can cause permanent vision loss if not treated quickly.
Certain patients need to be especially attentive to new or changing visual symptoms. People with diabetes who notice inconsistent vision should seek prompt evaluation, as this can be an early sign of macular edema. Anyone with a known history of AMD in either eye who experiences new blur or distortion should contact their Retina Specialist, since this may signal a conversion from dry to wet AMD. Patients with cardiovascular risk factors such as high blood pressure, high cholesterol, or a history of smoking should treat any episode of transient vision loss as a medical warning sign.
Not every episode of blurry vision requires an emergency visit. Blurriness that develops after long stretches of screen time, improves right away with blinking, and is not paired with any other visual changes is more likely related to dry eye or eye strain. Mild fluctuations that have been stable and previously evaluated do not require emergency reassessment. That said, any new pattern of fluctuating vision, or a change in a symptom that was previously stable, should always be discussed with your Retina Specialist.
How We Evaluate Fluctuating Vision
When you come to us with this symptom, our goal is to find the underlying cause quickly and accurately. We use a combination of clinical examination and advanced imaging to get a complete picture of your retinal health.
A thorough dilated eye examination is always the starting point. By widening the pupil with eye drops, our Retina Specialists can use specialized lenses and an indirect ophthalmoscope to examine the macula, the peripheral retina, and the vitreous in detail. This allows us to look for signs of fluid, hemorrhage, traction, retinal tears, or other structural changes that may explain your symptoms.
Optical coherence tomography, or OCT, is one of the most important tools we use to evaluate fluctuating vision. This imaging technology creates detailed, cross-sectional pictures of the retinal layers and can detect even small amounts of fluid that would not be visible during a standard exam. OCT helps us identify macular edema, subretinal fluid from wet AMD or CSC, and changes at the vitreous-retinal boundary. We also use OCT angiography, a non-invasive version that maps retinal blood flow without the need for a dye injection.
When we suspect a vascular or leakage-related cause, fluorescein angiography provides important additional detail. A fluorescent dye is injected into a vein in the arm, and a specialized camera photographs the retinal blood vessels as the dye passes through them. The resulting images reveal areas of leakage, blockage, or abnormal new vessel growth. This test is especially useful for identifying the source of fluid in DME and for confirming the abnormal vessels associated with wet AMD.
If the pattern of vision loss suggests a vascular cause such as amaurosis fugax, we may coordinate additional testing with your other physicians. This can include carotid artery imaging to look for blockages, blood tests for inflammatory markers associated with giant cell arteritis, and cardiovascular assessment. For patients with diabetes, your blood sugar control and recent lab work provide important context for managing diabetic eye disease effectively.
Conditions We May Diagnose
Depending on your examination and imaging findings, one or more of the following diagnoses may explain your fluctuating vision. Each condition has a distinct appearance on imaging and a tailored approach to treatment.
DME is confirmed when OCT shows fluid buildup in the macular area in a patient with diabetic retinopathy. The severity is assessed based on how much fluid is present and how close it is to the center of the macula. Early DME, where fluid has not yet reached the very center, often produces the kind of intermittent blurriness patients describe before noticing a persistent change in vision.
Wet AMD is identified when imaging shows abnormal choroidal neovascularization (new blood vessel growth beneath the retina) with associated fluid or hemorrhage. Detecting wet AMD at an early stage, when fluid is minimal and vision may still be fluctuating rather than consistently reduced, gives us the best opportunity to preserve central vision with timely treatment.
CSC is diagnosed when OCT shows a well-defined pocket of fluid beneath the macula with no evidence of abnormal blood vessel growth. Fluorescein angiography may reveal a pinpoint leak from the choroid. The characteristic pattern of vision clearing and then blurring again over weeks or months is particularly consistent with this condition. Most acute cases resolve within three to four months, though some become chronic or keep coming back.
VMT is identified on OCT as a persistent attachment between the vitreous and the macula, often with visible distortion of the macular surface. The extent of the attachment and the degree of distortion help our team determine whether watchful waiting or intervention is the right approach. Mild cases may resolve on their own as the vitreous eventually separates completely.
Treatment Options
Once we identify the cause of your fluctuating vision, we create a personalized treatment plan. The right approach depends on the specific condition, its severity, and your overall eye and medical history.
Anti-VEGF therapy is the standard first-line treatment for both diabetic macular edema and wet AMD. These medications work by blocking a protein called vascular endothelial growth factor (VEGF), which drives abnormal blood vessel growth and fluid leakage. Options include bevacizumab (Avastin), ranibizumab (Lucentis), aflibercept (Eylea), and faricimab (Vabysmo), injected directly into the eye in a procedure that takes only a few minutes. Treatment typically begins with a loading series of monthly injections, followed by an individualized maintenance schedule based on how your eye responds.
For patients with diabetic macular edema who do not respond adequately to anti-VEGF therapy, intravitreal corticosteroid implants offer an alternative. The fluocinolone acetonide implant (Iluvien) is a long-acting sustained-release device that delivers medication continuously over an extended period, reducing the number of required office visits. Your Retina Specialist will weigh the benefits and risks, including the potential for elevated eye pressure and cataract progression, when considering this option.
Acute CSC is usually managed with careful observation since most cases resolve on their own within three to four months. If corticosteroid medications are contributing to the condition, working with your prescribing physician to reduce or stop them may help. For chronic or frequently recurring CSC, photodynamic therapy using a medication called verteporfin can treat the underlying choroidal abnormality and reduce the chance of recurrence. Micropulse laser is another option for chronic cases and avoids the retinal scarring sometimes associated with conventional laser treatment.
When VMT is mild and causes minimal symptoms, we typically monitor it with periodic OCT scans to see if the vitreous completes its natural separation. If traction is significant and affecting vision, pars plana vitrectomy (a surgical procedure to remove the vitreous gel) can relieve the pulling force on the macula. In select cases, an enzymatic injection may be used to dissolve the attachment between the vitreous and the macular surface without surgery. Our team will discuss which approach fits your situation.
When transient vision loss points to a vascular cause such as amaurosis fugax, the priority shifts to reducing the risk of stroke and further vascular events. Treatment may involve antiplatelet medications, blood pressure and cholesterol management, and lifestyle changes. If imaging reveals significant narrowing in the carotid artery, a vascular surgeon may recommend further intervention. Not necessarily. With conditions like wet AMD and DME, the amount of fluid can temporarily decrease and then build back up, giving a false impression of improvement. With CSC, the fluid may truly reabsorb, but the underlying tendency to leak can cause recurrence. And with transient vision loss from a vascular cause, the fact that vision returned does not reduce the risk of a future, potentially more serious event. A return to normal vision does not replace a proper evaluation.
Treat this as a medical emergency, even if vision fully returns within minutes. Transient complete vision loss in one eye, known as amaurosis fugax, can be a warning of an impending stroke caused by a clot from the carotid artery. Time matters in these situations. Seek same-day evaluation at an emergency setting or contact your Retina Specialist immediately so that the appropriate urgent workup can be arranged.
Not necessarily. Many patients with DME or wet AMD start with more frequent injections during a loading phase and then transition to a longer interval as their condition stabilizes. Some patients achieve good disease control on an every-two-month or longer schedule. The goal is always to use the minimum frequency that keeps your retina stable, and that is reassessed at every visit using OCT imaging to track fluid levels over time.
Many patients are referred by their general eye doctor or optometrist, but a referral is not always required to schedule an evaluation.
